Some or all of the following forms will need to be completed, depending on your individual circumstance. These forms and information will be used to create your sacramental records, etc. Please look over the italicized section at the end to see which of these forms you will need to complete.
Information Form (IF): This is your basic information including contact numbers and e-mail. IF is to be completedby anyone who will be entering the Church or receiving any of the sacraments.
NOTE: This form asks for a Saint Name for Confirmation. The saint name will need to be chosen by about a month prior to the Easter Vigil. Please turn in this form as soon as possible and when a saint name is chosen, inform the RCIA contact of your choice.
Baptismal Certificate (BC): This is proof of Christian baptism. It does not have to be a certificate as many non-Catholic churches do not provide certificates. It can be a copy of a baptism register of the church, a letter from the church stating the baptism info, etc. If you have any questions, please check with the RCIA contact.
BC is to be providedby anyone who will be entering the Church or receiving any of the sacraments. NOTE: If you are NOT baptized, no form is needed.
Sponsor Form (SP): This form is for your Confirmation sponsor to complete. Once you have chosen a sponsor, please give this form to him/her to have completed and signed by their parish priest.
SP is to be completed by the sponsor of anyone who will be confirmed. This includes those entering the Church and any other person who is to be baptized or confirmed at the Easter Vigil.
Nullity of Form (NF): This form will not apply to most candidates. If you or your spouse have been married previously, it is important to disclose this information to our chaplain (the priest who is overseeing RCIA for the year). There may be additional steps that will need to be completed prior to any sacramental reception.
NF is to be provided if you have previously received an annulment from the Catholic Church or if the priest determines that you have to proceed with the annulment process.
